Field Trip Waiver

I understand that the student is not mandated to participate in the field trip.

I understand that the field trip may be subject to cancellation due to weather or safety concerns, in which case the school will notify me.

I understand certain risks are inherent in field trips. 

I understand that the school is not responsible for personal injury or loss to person or personal property that may occur during the field trip. 

I hold harmless and waive any claim against the school, teachers and staff for any loss or injury that may occur during the field trip. 

I give consent to the school to take pictures or video of my child during the field trip for the purpose of education, news releases or publicity. 

Should they deem the student requires medical treatment while on the field trip, the school will seek appropriate treatment from a physician, dentist or hospital. In this case every reasonable effort will be made to contact the parent/guardian. The school take no responsibility for any medical expenses, injury or damage that is incurred, and the parent/guardian will be fully liable for paying all costs of treatment.

The school will not be held responsible for the loss or theft of personal belongings of the student.

I have notified the school of any allergies or medical requirements that may be relevant to the trip.
