I am not under the influencer of alcohol and drugs.

I understand that tattoos are a permanent change to the skin, and I do not have any expectation that they can be reversed or modified at a later date.

I take full responsibility for the meaning and spelling of the tattoo text or design.

I have notified any medical conditions that would impair my ability to have or heal from a tattoo, including diabetes, haemophelia, eczma or psoriasis.

I acknowledge that with any tattoo there is a the possibility of injury such as infection, allergic reaction to the pigment, scarring, and others. And I understand that certain of these risks are increased if I do not take proper care of the tattoo.

I understand that the color of the tattoo is dependent on my skin and therefore may be different than my expectation, and may change over time.

I release Toni's Tattoo Shop and the Tattoo Artist for personal injury, property damage, or any other claims that may arise out of any negligent acts resulting from the application of my tattoo.
